The authors present a systematic review on the advantages and disadvantages of various brow-lifting techniques. They present their decision algorithm for closed versus opened approach, including potential complications and their management in general. After a detailed description of the endoscopic brow lift procedure, cosmetic and reconstructive indications together with definite benefits and risks of nonendoscopic approaches are illustrated. The following traditional techniques are discussed: coronal, peritrichial, direct, midforehead and transblepharoblasty approach. Although the endoscopic brow approach has been criticised for lack of long-term results, no high evidence level trials exist that directly compare the longevity of postoperative results gathered by the endoscopic approach with any of the traditional open approaches. In summary, the authors suggest that endoscopic brow lifting alone or combined with a trichophytic skin resection can be used to achieve excellent brow and periorbital rejuvenation in most patients with minimal complication and excellent longevity.
